Insurance Policy
Bayside Counseling accepts most insurance, including United Heath, Blue Cross and Medicare. It is your responsibility to know your insurance policy and benefits and to obtain prior authorization for treatment if this is required by your plan. Bayside Counseling, LLC will bill your insurance for your treatment sessions. Co-payments are collected at each session.
Fee Policy
You are responsible for any fees not covered by insurance.
Cancellation Policy
A missed appointment fee will be charged for appointments not cancelled twenty four hours in advance. There is flexibility for emergencies and acute illness.
How to make an appointment:
Appointments may be scheduled by phone during normal business hours (Monday-Friday 9:00am-5:00 pm). The office number is (401)263-3003. An intake form can be downloaded from this site, completed and brought to your first appointment or can be done at the time of your appointment. Please arrive several minutes before your first, scheduled appointment to complete the required paperwork. Every effort will be made to be flexible in accommodating your schedule.
